Abstract

Title: Role of Chest Computed Tomography in the Diagnosis of SARS-CoV-2 Pneumonia in Children and Adolescents

Background:
First PCR-confirmed COVID-19 cases among Russian children were reported in early March 2020. Chest computed tomography is the most sensitive imaging modality in diagnosing early signs of viral lung damage. Limited data are available on clinical and laboratory features of COVID-19 in children and adolescents, as well as on their correlation with imaging findings. Studying COVID-19 features in children and adolescents is crucial for modern medicine, which makes our research relevant.
Objective:
To determine the diagnostic efficacy of and indications for chest computed tomography in children and adolescents with COVID-19.
Materials and methods:
From February 2020 to February 2021 372 patients with suspected COVID-19 were admitted to Morozov Children’s City Clinical Hospital (Moscow, Russia). This retrospective study included 158 patients who presented with positive oropharyngeal and nasopharyngeal swab results, IgM and IgG test results, changes in complete blood count, blood biochemistry and coagulation test results, clinical symptoms, and suspected SARS-CoV-2 pneumonia based on chest computed tomography findings.
Results:
Chest computed tomography sensitivity and specificity for diagnosis of SARS-CoV-2 pneumonia were up to 98% and 85%, respectively. We considered that severe respiratory motion artifacts in young children under general anesthesia adversely affected the diagnostic efficacy. Sensitivity and specificity in this group were up to 89% and 71%, respectively.
Conclusions:
Viral pneumonia signs on chest computed tomography scans were statistically significantly more frequent in children with cough, respiratory failure signs, lymphopenia, and increased D-dimer and C-reactive protein levels. One of these signs or a combination of them are an indication for chest computed tomography in children with suspected SARS-CoV-2 pneumonia.

Abstract

Title: Cancer Patterns among Patients Admitted to the Oncology

Background: Cancer is a major public health problem that faces health managers and decision-makers in many countries. Cancer is constantly growing and is one of the major causes of mortality in Libya.
Aim: To estimate the geographical distribution and patterns of cancer among patients admitted to the Oncology Department at Benghazi Medical Center (BMC).
Methods: A retrospective cross-sectional study was conducted from March to April 2019. A total of 4649 patients’ files with a diagnosis of cancer at the BMC were extracted as hard copies due to the absence of electronic medical files. A sampling approach was performed to facilitate the analyses. The sample size was (206) files out of (4649) files, which was determined via a statistical software program (Epi Info7) Data was collected retrospectively from patients’ files; on age, gender, address, and classification of the cancer diagnosis which was made according to (ICD–Oncology) for a specific period (5 years – from January 1st, 2013 to December 31st, 2017).
Results: The results revealed that more than half of the cancer cases admitted to Benghazi Medical Center were from Benghazi City and (52.4%) of cases were females whereas (47.6%) were males, (18.9 %) were (65y). The most common cancers among females were breast cancer (28.6%) and endometrial cancer (7.3%), whereas among males were colorectal cancer (8.3%), prostate cancer (6.8%), gastrointestinal (6.3%) and lung cancer (6.3%).
Conclusion: The most common cancer among women was breast cancer and among men was colorectal and prostate cancer. The majority of cases were from Benghazi. Further studies are required to confirm such results. Educating people about the risk factors that could increase cancer incidence, especially lifestyle, and dietary patterns. Naeima Houssein was born in1982 in Libya, she graduated from department of Environmental Health, university of Benghazi in 2007. Later in 2011 she got an MPH from university of Glasgow, the UK focused on Epidemiology. She shares this work with her colleague Fathi Omar who was born in 1977 in Libya, he graduated from the department of health administration, faculty of public health in 2004. and he got his Master degree in health care management from university of New Haven, USA in 2010.

Abstract

Title: Predictors of dehydration following adenotonsillectomy in Jordanian pediatric cases

Background: Dehydration is a well-established complication of adenotonsillectomy. This study aims to measure the prevalence of dehydration among pediatric adenotonsillectomy patients in a tertiary hospital in Amman and to identify the risk factors that could be associated with it.
Methods: This is an observational single-center study. Data were collected by reviewing the health records of patients who underwent adenotonsillectomy between January 2015 and June 2020 at Ibn Al-Haytham Hospital. Inclusion criteria were any patient between 1 and 12 years old that has undergone routine adenotonsillectomy. Exclusion criteria were any adenotonsillectomy for neoplasm purposes, patients with reported developmental delay, and patients who underwent adenoidectomy or tonsillectomy alone. Collected data included patients’ demographics, indication for adenotonsillectomy, type of surgical technique, and history of dehydration in the following two weeks post adenotonsillectomy. The data were then imported into an SPSS statistical spreadsheet and analyzed. Descriptive statistics analysis of the demographic characteristics of the cases was prepared. Numerical data were expressed as percentages or means ± standard deviation (SD). Results: Three hundred and eighty-four patients met the inclusion criteria of this study. 234 patients (62.2%) were male, and the majority of the cases (223 patients) were between 5 and 6 years old, accounting for 58.8% of the population. The prevalence of postadenotonsillectomy dehydration was 5.7%. Point estimation with a 95% confidence interval falls between 5.17 and 5.63. Dehydration was more prevalent in children aged under three years old. Dependence-type Multivariate analysis revealed that age and gender remained significantly associated with dehydration with P values > 0.001 and 0.004, respectively, after adjusting for the other variables.
Conclusion: Dehydration is a serious yet rare complication post adenotonsillectomy. Screening for dehydration pre- and postdischarge is highly recommended. There is a need for further multi-center and population-based studies to examine the full extent of dehydration complications. It is in the best interest of surgeons and all caregivers to provide the best quality of care for adenotonsillectomy cases. Avoiding dehydration and all other surgical complications would be part of the standards of high-quality healthcare.

Abstract

Title: Systematic monitoring of the cardiovascular risk in the primary care approved for the Slovak Republic

Systematic monitoring of the cardiovascular risk (CVR) is the standard program of the primary prevention approved for the Slovak republic by the minister of the health by July 2022.  It consists of the combination of the cardiovascular risk factors monitoring at the levels of GPs, health insurance companies and national registry, together with positive economic stimulation to decrease the risk factors levels in the population.  The aim is to reduce cardiovascular morbidity and mortality and prolong averaged value of healthy life.

In the program at the GPs level are included:

1. Men aged 40-65 years on the regular preventive examination every 2 years
2. Women aged 50-65 years on the regular preventive examination every 2 years
3. Patients less than 70years old with the chronic inflamatory diseases, erectile dysfunction or with  migrain accompanied by aura.

Basal parts of the program are:

• Physical examination, weight and height for the BMI calculation, waist circumference
• CVR calculation according to SCORE2
• Modelling of the possible risk factors changes and consequent SCORE2 values in the interaction with examined person
• Person assignment into the adequate CVR group
• Regular passing of the counted individual SCORE2 values into the health insurance companies together with values of BMI, waist circumference and odd parameters used for SCORE2 evaluation
• Regular passing of these parameters into the National centrum of health information.

Every health insurance company counts for the every GP on the 2 years intervals basis averaged values of the CVR, BMI, waist circumference measured from all the persons included in the program after adjusting on age, sex and life environment.

Award will be provided to

• Every person who was able to reduce CVR value at the next preventative examination after 2 years at least 2,5%
• Every person who has had waist circumference at the high metabolic risk level (more than 102 cm man resp. 88 cm women) and who was able to reduce these values into the level of increased metabolic risk (94-101 cm resp. 80-87)
• Every GP who has reached averaged values of the CVR, BMI, waist circumference measured from all the persons included in the program after 2 years at least 2,5% less than before
• Award form and maximal possible award value are determined by every health insurance company, minimal possible award value determines every year Ministry of Health both for persons on the regular preventative examination and GPs.

Therefore, Islamic Sharia has given great attention to it due to its importance in shaping personality and building correct behavioral patterns. Sharia has considered the inclinations and instincts within humans, seeking to organize, refine, and guide them correctly. It has focused on innate disposition and relied on it to achieve integration between physical, spiritual, psychological, and social aspects, making it a principle in respecting legislation and the dominance of its rulings. The standard of righteousness, success, corruption, and failure, by which systems and laws are measured, is based on fully considering this innate disposition. Achieving comprehensive healthcare balance, enhancing mental stability and healthy behavior, cultural awareness of the importance of self-care and personal and societal responsibility in maintaining health, achieving environmental balance, and making appropriate decisions, all stem from innate disposition and align with what Islamic Sharia has brought in many practical and applied health examples. These have contributed to providing an ethical and spiritual framework that has improved human appearance, behavior, health, and community building. Mr. Mohammad Subhi Hamed Odeh, a university lecturer at the Faculty of Sharia, An-Najah National University, Nablus, Palestine. He is currently pursuing his doctoral thesis and has conducted several scientific research studies, including the pathway of considering innate disposition and its impact on achieving the objective of respecting legislation, selling human blood, donating it, and the consequent effects in Islamic jurisprudence, and preventive measures to limit the spread of the coronavirus.

Dr. Nasser Al-Din Muhammad Ahmad Al-Sha’er, a university professor at the Faculty of Sharia, An-Najah National University, Nablus, Palestine. Former Palestinian Minister of Education and Higher Education. He holds a PhD from the Department of Middle Eastern Studies at the University of Manchester in Britain. He has published numerous scientific research papers, including globalization, violence against women, and reproductive health. Among his authored books are “Scientific Research and its Applications in Humanities and Sharia Sciences,” “Family System in Islam,” “Islamic Culture,” “Social Communication,” and “Human Rights in Higher Religious Education Curricula.” He has participated in various international scientific, cultural, and social conferences, seminars, and workshops.
