Title: Pregnancy outcome in cancer survivors

Introduction: Advances in cancer detection and treatment have resulted in increased prevalence of female cancer survivors at child-bearing age-group. However, data regarding pregnancy outcome of cancer survivors are scarce.

Aims and objectives: This article aims to determine whether cancer survivors have increased risk of obstetric complications.

Results: We identified eight patients who have been treated for various types of malignancy and their maternal and fetal outcome during pregnancy. We identified the outcomes and possibilities of various diseases like choriocarcinoma, gestational trophoblastic neoplasia, synovial cell carcinoma, bronchogenic carcinoma, acoustic schwannoma, papillary carcinoma thyroid, chronic myeloid leukemia (CML), and cystadenocarcinoma of ovary.

Conclusion: As developments in oncology continue, it is likely that we will see an increasing number of women with a history of cancer within obstetric services.

Title: Identity Effects of Differential Gender Socialization on Young Adolescents’ Aspirations for First Child and Marriage in Ouagadougou: A Mixed-Methods Study

Previous studies show that gender socialization develops differentiated gender identities in girls and boys, influencing their self-esteem and sexual behaviors. Although early adolescence is considered a pivotal period for gender socialization, few studies in Francophone Africa investigate the role of gender identity effects from this process on sexual and reproductive health aspirations and behaviors among this population. This article uses data from young adolescents and parents in Ouagadougou to examine how the outcomes of gender socialization among girls and boys are associated with their aspirations for marriage and having their first child. The results show that young adolescents adhere to unequal gender norms. This leads to girls having earlier aspirations for marriage and later aspirations for childbearing, while boys have earlier aspirations for childbearing and later aspirations for marriage. These differential effects can expose girls and boys to risks of poor sexual and reproductive health. Interventions promoting gender-equitable norms could strengthen girls’ self-esteem and mutual respect between the sexes, thereby improving their sexual and reproductive health during youth and adulthood.

Title: To BE or Not to B: the application of Uterine Compression sutures to Reduce Blood Loss after myomectomy.

Introduction: The aim was to assess the hemostatic impact of B-Lynch sutures following an open myomectomy for efficacy.

Material and methods: In this prospective clinical research, performed in Alazhar University Hospitals (Al-Hussain, Damietta, Assiut) and Minia University Maternity Hospital, 250 women scheduled for open myomectomy between January 2021 and January 2023 had multiple fibroid uteri with uterine sizes corresponding to 12–22weeks. There were two groups of women. Group I (125) underwent standard open myomectomy surgery, whereas Group II (125) underwent normal open laparotomy surgery followed by B-Lynch sutures. Certain inclusion and exclusion criteria were applied to every patient. We recorded vital data, length of the procedure, complications (bleeding during the procedure, bleeding from multiple bites, bladder injury, fever, wound infection), complete blood count before and after surgery, need for blood transfusion, postoperative vital data, time until ambulation, passing flatus, and ability to eat and drink, as well as the amount of blood lost during and after the procedure.

Results: There was no statistically significant difference between the two groups in age, parity, weight, number of fibroids, or uterine size as measured by ultrasonography. Between groups I and II, there was a significant difference in the average intraoperative blood loss (Group I lost 562.6 ml, whereas Group II lost 411.3 ml) as well as the mean blood loss following surgery (205 ±82 ml in Group I and 117 ±41 ml in Group II). No significant difference was observed in the mean length of hospital stay between groups I and II (2 ±0.3 days and 2 ±0.6 days, respectively).

Conclusions: Using a B-Lynch suture can help minimize blood loss during and after an open myomectomy. Therefore, if the uterus is large and has a lot of fibroids, it is recommended to be done frequently.

Title: Women’s experiences and perceptions on the impacts of maternal near miss and related complications in Rwanda: A qualitative study

Maternal morbidity and mortality continue to emerge across the globe especially in lower-income countries. This study aimed at exploring in-depth perceptions of near-miss experiences among Rwandan women and how these experiences can be used to develop strategies for health policy implementation. Using qualitative inductive research based on grounded theory, we analyzed 27 in-depth interviews that were conducted with women with documented records of maternal near-miss events. Women were knowledgeable about pregnancy complications and the benefits of antenatal care. Near-miss events that occurred either before or during hospitalization. Women recognized their own involvement their near-miss events by delaying care seeking. They also mentioned delays due to healthcare providers delaying transfers, misdiagnosing the events, and delaying to intervene even at the time the diagnosis was made. Women acknowledged the life-saving role of outreach programs and community health workers. We believe that pregnancy outcomes would be improved in this population of women with education on pregnancy complications, training of community health workers, and sustained mentorship program.

Title: Prevention of preeclampsia and its relevance in the Latin American context: a narrative review

Preeclampsia is a hypertensive disorder of pregnancy that is evident during the last weeks of it. Globally, preeclampsia is the second leading cause of maternal mortality. It is estimated that the incidence is approximately 16% in developing countries, and reaches over 25% in some Latin American countries. The main strategies for the prevention of preeclampsia and their relevance in the Latin American context will be described. The search for information took place in PubMed and BVS for publications on preeclampsia prevention, using the search expression (“Disease Prevention” OR “Primary Prevention”) AND (Pre-eclampsia OR Pre-eclampsia OR “Pre eclampsia”) AND (“Latin America” OR “Central America” OR “South America” OR Caribbean). In PubMed we found 60 and in VHL 42 results that respond to the search expression, and 18 articles were selected that discuss prevention strategies for preeclampsia in Latin America. With these results, it is possible to afirm that describing the best strategies for the prevention of preeclampsia in Latin America is a pending task, since there is a relative lack of publications. Consequently, it is hoped that this review will motivate future research and be a useful resource in medical updating.

Title: After the Alabama Supreme Court decision, the futuristic view of the Reproductive Hierarchy

The separation of sex and hi-tech reproduction treatments led to a revolution of gender and similarity relations, while embryo diagnostics led to a shift from scheduling families to planning a child. Furthermore, the presence of fertilized eggs outside the womb is a new form of human life which can be conserved and manipulated and the embryo in the petri dish in laboratory has become the entity of reproductive market. This presentation firstly presents embryo moral approaches briefly, especially regarding interventions in preimplantation-stage embryos in the laboratory, suggesting environmentally suitable laboratory conditions for this entity. Additionally, after Alabama supreme court decision, new suggestions for legitimately suitable regulations takes care precisely embryo’s right, but also embryology laboratory personnel, clinics, and parents.

Dr. Bahar Uslu is a Board-Certified Specialist in Histology and Embryology, and the founder and former director of Reproductive Medicine, Andrology, and Infertility Units, where she served for 24 years. She completed fellowships in reproductive endocrinology and infertility at Yale University. As the Founding President and current Vice President of the Society of Histologists and Embryologists (SOHE), Dr. Uslu actively mentors students and researchers, including at Yale and Quinnipiac.

She is known for innovating the “Uslu Ovarian Follicle Counting Method,” published in the Journal of Ovarian Research in 2017, and has been recognized as one of the best reproductive medicine specialists, earning accolades including the U.S. Government’s recognition for “Extraordinary Skill in Medicine.”

Title: Association between emotional and social support and cervical cancer screening among Mississippi women

Background Mississippi’s age-adjusted cervical cancer mortality is the second highest in the nation (3.4/100,000). Disparities in cervical cancer mortality by race continue to persist in Mississippi. Cervical cancer mortality among non-Hispanic Blacks is higher compared to non-Hispanic Whites (3.7/100,000 vs 3.2/100,000) respectively. Cervical cancer is preventable through routine screening according to the recommended guidelines. Research has shown that emotional and social support may facilitate screening but research is limited regarding Pap test uptake among Mississippi women. This study was conducted to examine the association between emotional and social support and Pap test uptake in Mississippi.

Methods A secondary analysis of data from the 2022 Behavioral Risk Factors Surveillance System (BRFSS) was conducted. As BRFSS is a complex survey design, data was analyzed using proc survey frequency for categorical variables, and proc survey mean for continuous variables. Proc survey logistic regression was performed to report odds ratios adjusting for sociodemographic characteristics.

Results Findings suggest that emotional and social support is significantly associated with cervical cancer screening (p=0.0013). Non-Hispanic Black women who reported no or limited emotional and social support are disproportionately not being screened for cervical cancer compared to non-Hispanic White women (62.5% vs 47.9%) respectively.

Conclusion Based on preliminary data, among Mississippi women, emotional and social support may be associated with cervical cancer screening. Public health practitioners should focus on interventions that target social support networks that may help increase cervical cancer screening and adherence to the recommended guidelines in Mississippi.

Mauricio Reyna Lara holds a Doctorate in Law with honors from the National Autonomous University of Mexico, as well as a Doctorate in Pedagogy from the Faculty of Philosophy and Letters at the same institution. He earned a Master’s degree in Law with honors from the National Autonomous University of Mexico and is a Specialist in Constitutional Law from the University of Salamanca, Spain. He has also completed research stays at the University of Salamanca, Spain, and the IRI Research Center in Germany and Switzerland. In October 2022, he was recognized as a Level I National Researcher by the National System of Researchers (SNI) of CONAHCYT.

Achievements:

These have been across various federal public administration entities, federal organizations, and both public and private institutions. He has been an educator and guest speaker at many institutions. Title: Undergoing Perimenopause: A look at the experience in women with systemic autoimmune diseases and fibromyalgia

Abundant research has explored the physiologic changes surrounding menopause including an overall trend toward increased inflammation. Accordingly, in most rheumatologic diseases, while selected features may abate, symptom exacerbation is more common, as precipitated by the dynamic changes that take place with the onset of menopause. Similarly, in fibromyalgia, levels of pain and wide-ranging somatic symptoms tend to intensify with menopause. Despite these trends, however, the intricacy of the intertwining psycho-neuroendocrine-immune pathways results in a much more complicated picture with often unexpected clinical manifestations. Furthermore, the comorbidities accrued by increasing age and the consequences of long-term medication use may also confound the clinical picture. Routine rheumatologic care of women with systemic autoimmune and fibromyalgia-related conditions involves regularly scheduled visits. Nonetheless, repeated publications highlight a gap in medical professionals’ educating and addressing issues concerning the impact of menopause. Moreover, few studies have explored how to optimize the treatment of chronic diseases through the menopausal transition. In the last several years, global attention toward menopause and its effect on working women has burgeoned. Only with focused scientific studies that integrate advanced learning technologies with a holistic and more personal approach toward treatment, will medical practitioners be able to optimize care, smoothing out the course of menopause for women.

Abstract

TITLE: Bedtime massage intervention for improving infant and mother sleep condition: A randomized controlled trial

Infants’ sleep disorders and parents’ insufficient sleep are common problems in infant care. The current study was conducted to assess the effectiveness of infant massage on infants’ night-time sleep condition and mothers’ sleep quality.140 infants were randomly put into two groups: the experimental group with fifteen-minute bedtime messages for two weeks and the control group with normal infant routine care. The Brief Infant Sleep Questionnaire, a personal information submission form, and the Pittsburgh Sleep Quality Index for the mothers were used to gather data in this study. Infants in the experimental group showed meaningful differences in variables such as sleep latency (P < 0001, eta = 0.099), number of night waking (P = 0.03, eta = 0.027), and longest continuous sleep period (P = 0.03, eta = 0.026). As for other variables, no meaningful differences were observed. There wasn’t a meaningful difference in the mother’s overall night-time sleep quality between the two groups (P = 0.184, eta = 0.012) except for the duration of the mother’s night-time sleep (P = 0.028, eta = 0.026) and the reduction of maternal sleep disorder (P = 0.020 eta = 0.029). The findings indicated that infants’ bedtime massages would improve some of the sleep markers of mothers and infants, and therefore, can be suggested as a practical, harmless, and cost-free method to improve sleep.

Abstract

TITLE: Case report: Success story of acute abdomen in pregnancy – Torsion Ovary

Torsion ovary is one of the acute emergencies in pregnancy with a prevalence of 2.7-3% with high maternal and fetal mortality rates. Ovarian torsion is the complete or partial rotation of the adnexa along its axis. Delay in diagnosis leads to hemorrhage, infarction, and necrosis resulting in loss of ovary. Although 26% adnexal torsion can occur in apparently healthy adnexa, the exact etiology remains unknown, predisposing factors include larger cyst size, free mobility, and long pedicle. Acute onset of pelvic pain followed by nausea and vomiting is the most common presenting symptom. High levels of clinical suspicion along with ultrasound and Doppler aids in diagnosis. We present a case of 18 weeks multipara with acute abdomen diagnosed as ovarian torsion -emergency laparotomy was done and scalping-oophorectomy proceeded given tortured necrosed ovary without disturbing gravid uterus. Pregnancy was continued and an alive term baby was delivered by normal vaginal delivery. Once torsion ovary is suspected, surgical management remains the treatment of choice.

Kalyani Saidhandapani is a DGO/DNB(OBG)/Mnams(OBG) Laparoscopic & Endoscopic Surgeon and Medical Director & Chief Gynaecologist/HOD (OBG Department) in Southern Railway HQ Hospital, Chennai. Her achievements are given top paper presentations at OGSSI 2007 and YUVA FOGSSI in 2011 and 2014. She won an award for an intriguing case report at IRAGO 2008 and has presented papers at national conferences. Additionally, she has published 10 articles in international and SAFOG journals. Her paper has been chosen for the international SAFOG conference in May 2024 in Bangkok, and she will present at the World Congress in August 2024.

Abstract

Title: Association Between Appropriate Prenatal Control Andmaternal Factors in Women Attended in the Mother Infantil Hospital Ramon Sard During the Year 2018-2019

OBJECTIVES: To evaluate the partnership between compliance with proper Prenatal Control and some maternal factors.

MATERIAL AND METHODS: Analytical design, cross-sectional in 340 women. A prenatal card and a closed survey were used to obtain precocity, periodicity, and completeness information in women who took their immediate postpartum during May 2018- February 2019. Variables: Appropriate Prenatal Control, Maternal Age, Parity, Educational Level, and Place of Residence.

RESULTS: Of 340 women, 19.12% had adequate prenatal care. 30.88% managed to start early; 60.59% were periodic and 39.12% was complete. 29.70% did not meet any of the 3 requirements. We do not find a significant difference for proper prenatal monitoring and parity, observing an OR-0.65; IC95%: 0.37-1.12, either with maternal age, with an OR 0.84; IC95%: 0.38-1.81, as well as the instruction level with an OR-0.59; IC95%: 1.17-2.02. For the place of residence, we find an OR 3, 03; IC95%: 1.73-5.32 giving statistically significant.

CONCLUSION: The percentage of women who meet the requirements for adequate prenatal care is significantly low. There is a pending debt with 101 (29.70%) who did not meet or fail to meet any requirement, considering that health care during pregnancy is a good example of preventive medicine in the perinatal field.

Abstract

TITLE: Effect of Fast-Track Surgery under ERAS protocol in Laparoscopic Hysterectomy: A Randomized Controlled Trial

Background & Objective: Fast-track surgery (FTS) consists of different pathways to decrease surgical complications and improve outcomes and patient satisfaction. FTS in an elective gynecologic laparoscopic surgery has not been well assessed. No consensus guidelines have been developed for gynecologic laparoscopic surgeries. The purpose of this study is to evaluate enhanced recovery after surgery (ERAS) for total laparoscopic hysterectomy. Materials & Methods: 260 patients underwent for laparoscopic hysterectomy surgery. All patients were divided into two groups as follows: one group received traditional laparoscopic hysterectomy surgery and the intervention group was treated under ERAS protocol. ERAS protocol includes not receiving preoperative mechanical bowel preparation and laxatives as well as fasting 6 h and not drinking liquids 2h before surgery. The patients were allowed to resume the ordinary diet 6h post-operation. Ondansetron 4 mg were prescribed after surgery for nausea and vomiting, pain was controlled with non-narcotic analgesics diclofenac suppository 100 mg/q12 h and paracetamol 1000 mg/q6 h until discharge. Urinary catheter was removed whenever possible and early ambulation occurred 6h after the surgery

Title : Birth and birth of immigrants in hospitals in the maule region

Objective: the purpose of this study is to describe the delivery and births of immigrants in the hospitals of Linares, Cauquenes, Constitución and San Javier in the Maule region, in the years 2020 and 2021.

Material and method: a descriptive cross-sectional study was carried out through the analysis of secondary databases of the records of public hospitals in the Maule region in the years 2020 and 2021. The communes corresponded to Linares, Cauquenes, Constitución and San Javier. The sample size corresponded to 4,870 birth records in the region for the years 2020 and 2021. The study variables analyzed were the types of delivery (cesarean section or vaginal delivery) according to nationality and weight in grams.
