Abstract

Title: “Characteristics and Outcomes of Acute Appendicitis in Lautoka Aspen Hospital, Fiji: An Observational Retrospective Study (09/22 – 09/23)”

Acute appendicitis is a surgical emergency characterized by symptoms due to the inflammation of the vermiform appendix. Delay in treatment increases the incidence of complications.

Majority of the patients were in age range of 11 to 30 years of age (50.5%) with male predominance (56%). The mean duration of symptoms was more than 2 days. The most common presenting symptom was localized right quadrant pain (59%) while right lower quadrant direct tenderness was the most common sign (94%). Abdominal ultrasound was done for most patients (92%) with sensitivity and specificity of 81% and 33.3% respectively. Majority of patients had complicated appendicitis (60%) with negative appendectomy rate of 7%. The mean duration of hospitalization was 4.61 days. Complication rate of this study was 23.5% with surgical site infection being the most common (19%). The mortality rate for this study was 0%.

Acute appendicitis was the most common cause of abdominal surgery with a high prevalence rate in young adults. Abdominal ultrasound was done in the majority of the patients as supplementary imaging for the diagnosis of acute appendicitis. Empirical antibiotic treatment was given to all patients. Complicated appendicitis was the most common intraoperative finding while the most common complication was surgical site infection.

Abstract

Title: “A case report of systemic lupus erythematous presenting with diffuse alveolar hemorrhage”

Systemic lupus erythematous (SLE), a chronic autoimmune disease with a wide range of manifestations such as, skin rashes, arthritis, fatigue, neurologic symptoms, stroke, glomerulonephritis, cardiovascular symptoms, and involvement of the lungs. Diffuse alveolar hemorrhage (DAH) is not common among SLE patients, but the mortality rate of DAH associated with SLE is reported to be significant. We report a 35 years old woman that diagnosed systemic lupus erythematous presenting with diffuse alveolar hemorrhage.
The patient is a 35-year-old unmarried woman who came after a recent cold with complaints of shortness of breath at the level of MMRC II, cough, massive hemoptysis, skin lesions on the lower limbs and left elbow. In the initial paraclinical examinations, the patient had a drop in white blood cells and hemoglobin.

Abstract

Title: “Association of diet with abdominal obesity”

The aim of the current study was to investigate dietary association with
abdominal obesity. A qualitative research study was followed in this study. A total of 200 housewives between the age group of 25 to 40 were enrolled in the study as the sample. A proper questionnaire was developed prior to conduction of research study. The data for the study was collected using a validated food frequency questionnaire approved by the supervisor of the project.The results indicated waist to hip ratio for most of the respondents i.e., 56 percent was high i.e., 0.85 and above meaning that most of the respondents
were having abdominal obesity. Body Mass Index results indicated that most of the respondents i.e., 55.71 percent of the total respondents were overweight. Most of the respondents i.e., 45.71 were having very high levels of visceral fats, while most of the respondents i.e., 67 percent were involved in moderate physical activity.

Abstract

Title: “Metabolic Dysregulation and Its Role in Postoperative Pain among Knee Osteoarthritis Patients”

The study aimed to investigate the association between POP development and disturbances in energy metabolic conversions, focusing on carbohydrate and fatty acid metabolism. Methods: Peripheral blood samples were collected from 26 healthy subjects and 50 patients with end-stage OA before joint replacement surgery. Pain levels were assessed before surgery using the visual analogue scale (VAS) and neuropathic pain questionnaires, DN4 and PainDETECT. Functional activity was evaluated using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Six months after surgery, pain indices according to a VAS of 30 mm or higher were considered.

Title: “Factors Affecting Adherence to Drug Therapy of Patients with Stable Coronary Heart Disease in the Republic of Dagestan”

Aim of research. Study of adherence to drug therapy and factors influencing it among patients with stable ischemic heart disease in the Republic of Dagestan.
Materials and methods of research. A study was conducted on 85 patients hospitalized routinely from 1/01/2015 to 31/12/2017 with a diagnosis of “Ischemic heart disease. Stable exertional angina.” At the first stage of the study, data were collected from medical records for the period from 1/01/2015 to 31/12/2017. All patients, with whom telephone contact was established and who were alive, were invited for a re-examination to assess patient adherence to therapy on outpatient stage and factors influencing it, using specially developed questionnaires. Abstract

Title: “Clinical observation of a patient with primary hypoparathyroidism, uncontrolled hypocalcemia and heart failure”

Hypocalcemia and associated symptoms are common manifestations of postoperative hypoparathyroidism clinical picture. Meanwhile, there are not so many patients with hypocalcemia occurring due to primary hypoparathyroidism, manifested by heart failure (HF). Current article describes a clinical case of a 45-year-old female patient with primary hypoparathyroidism and uncontrolled hypocalcemia, in whom acute HF was developed, and was accompanied by a significant decrease in the left ventricular ejection fraction and heart cavities dilatation. After normalization of the calcium level in the blood serum, the clinical symptoms of HF regressed, cardiac function normalized. That rare case has shown the key role of calcium in maintaining cardiac function in such kind of patients, as well as the critical importance of abolition the reversible causes of HF.

TITLE: “A Rare Cause of Pulmonary embolism”

Cardiac pacemakers are progressively employed at higher rates within the realm of clinical cardiology to address diverse medical indications. Lead fractures represent untoward occurrences that may manifest in individuals under pacemaker surveillance. The occurrence of lead fractures may give rise to varying complications contingent upon their specific anatomical locations. In the context of this report, we illustrate a case involving a patient equipped with a cardiac pacemaker, in whom the identification of a lead fracture coincided with an investigation into the etiology of pulmonary embolism.

He has been working as a Consultant Cardiologist at Beyhekim Training and Research Hospital since March 2024. I completed my Cardiology Residency training at Necmettin Erbakan University Faculty of Medicine, Department of Cardiology, as of January 2024. During my cardiology fellowship, He served as head-fellow for 8 months. During my fellowship, He has been passed the European Exam of Core Cardiology and received the ESC Board certificate. Additionally, during this period, He served as an observer in Minnesota, USA. He made many oral presentations at home and abroad.
